Information obtained from this survey will be used to produce a new Citizen Monitoring directory and a Google map similar to the ones currently found on the Clean Water Team Website. A citizen monitoring report will also be created using information learned from this survey. Attached is a copy of the survey for informational purposes. The survey can only be completed "on-line" at the below website.
