FEMA Tribal Courses at NETC EMI/NFA

Hello All,

Many of you have requested the Schedules for the up and coming EMI Tribal Courses. OK, here is the List, the App Form (Attached: Adobe PDF File - FEMA Form 119-25-1), and how to fill out the form.

COURSE DATES:

E580 Emerg Mgt Framework for Tribal Gov. March 19 - 22, 2012

E580 Emerg Mgt Framework for Tribal Gov. Aug 20 - 23, 2012

E581 Emerg Mgt Ops for Tribal Gov. May 7 - 10, 2012

E582 Hazard Mitigation for Tribal Gov. May 21 - 24, 2012.


FILLING OUT APP:

Listed below is what needs the be addressed for each App Question:

QUESTIONS:

1. YES unless born out of Country.

2. List last name first, first name, middle initial as you would want to see your name on the certificate.

3. List your full Social Security Number.

4. List home address, street, city, state and zip code only.

5. List work phone number.

6. List cell phone number.

7. List your fax number if none leave blank.

8. List email address.

9a. List class number ie. E 580 class name.

9b. NETC EMI

9c. List first date class starts.ie. March 12, 2012.

10. For E 580 class list all related information required for attending the ICS 100/NIMS 700 Courses. For
all other classes list all required information for when you took the E 580 class.

11. Check yes or no as appropriate.

12a. List name of organization you work for and their mailing address on the second line.

12b. N/A

13. List current position and number of years in that position, ie. police chief 2 years.

14a. Check box number 9 (Tribal Nation)

14b. Check appropriate box.

15. Check appropriate box.

16. Describe what you do for your organization. How this class will benefit you and how it will benefit your
Tribal Community.

17a. Check appropriate box.

17b. Check appropriate box.

17c. List total number of years of government or service experiance.

17d. Total number of people who are Tribal members or who you work with.

17e. Check appropriate box, ie. number 5 for tribal cert group.

18. List date of birth.

19. Check proper gender box.

20a. Check proper box.

20b. Check proper box.

21d. Under box 21d sign your name and put date you signed in date box.

22a. Have your supervisor or Tribal Chairperson sign your application on 22b. print their name and title.

24a. This is the address that you will mail your application to in a 9 x 12 envelope that will include both
pages of this form and a third page with and organizational chart.
