Readiness Survey--Tribal Emergency Preparedness

You are invited to participate in this pilot research study because of your employment position and key role in the Tribal emergency preparedness workforce of your Tribe. As part of this pilot research study, we are asking key individuals from the Tribe to fill out the Tribal Emergency Preparedness Pilot Project Survey. As a participant you are asked to provide information on the readiness of your Tribe as it pertains to emergency preparedness. The survey will focus on four different aspects for emergency preparedness: Current preparedness level, funding resources, emergency preparedness resources, and plan to develop future preparedness. The purpose of the survey is to assess current preparedness and funding levels of Tribes throughout California and outline the possible challenges and gaps to improve Emergency Preparedness among Tribal Nations.

PROCEDURES: Participation in this pilot study involves taking 30-45 minutes to complete a survey. This survey is confidential and contains only two identifiers: the name of the Tribe and position of the individual completing the survey. The questions on the survey will pertain to four broad topics mentioned above, and include questions such as the following:
• How prepared do you believe your Tribe/Band is for an Earthquake?
• What are the sources for your current emergency preparedness funds?

POTENTIAL RISKS AND BENEFITS: The Loma Linda University Institutional Review Board (IRB) reviews all studies involving human participants. The Board has determined that participation in this study exposes you to minimal risk of breach of confidentiality. While you are unlikely to directly benefit, the overall outcome of this pilot project will highlight the challenges and gaps in Emergency Preparedness funding and resource allocation. We hope that this will foster inter-Tribal, local, state and federal cooperation for future emergency preparedness planning.

COSTS AND REIMBURSEMENT: There is no cost to you for participating in this study. If you complete the survey, you will receive a $25 Visa gift card as thanks.

CONFIDENTIALITY: The information will be kept confidential. The final report which will be published and sent out all the participating Tribes and Tribal organizations and will not include any identifiable information you provide on the survey.

VOLUNTARY PARTICIPANTION: Participation in this study is voluntary and you may refuse to take part, or choose to stop taking part, in this study at any time.
